Application procedures:

1. Obtain an application form. The application form for the summit is printed and distributed by the summit secretariat, which can be obtained from the summit secretariat by mail or telephone, or downloaded from the official website of the summit (http://www.cabiforum.org).
2. Fill in the application form and fax or e-mail the form to the summit secretariat before 30 September 2008.
3. All applications subject to confirmation by the summit secretariat, which will be sent to applicants by fax and e-mail.
